A Review of Transition from School to Community for Students with Disabilities in NSW, Australia
Transition is a process of changing from one state, activity or place to another. This article recognises that transition is a process and not a program. It identifies the critical features in effective transition, including the need for Individual Transition Planning (ITP). The article also touches on vocational training, school-based service provision, post-school options and the professional development needs of staff.
