This Report discusses the 'health planning' needs of families where children have complex medical needs. The social and policy trends that affect children with complex medical needs and their families are examined. These trends include shifting definitions of disability and philosophies of care.,,The Report discusses specific health planning issues and argues for an alternative health planning model based on community partnerships, flexible and responsive funding support and individual planning. The health supported model should be community based and guided by self-determination, accountability and equality of access. When communities are encouraged to recognise 'the art of the possible', children with complex medical needs can be effectively included in their families and communities.,
